San Gabriel/Pomona Regional Center relies on our service providers to meet the needs of our community. We are proud of our partnership with providers who seek to deliver high quality services to the individuals and families served by San Gabriel/Pomona Regional Center.

Our partners not only meet the challenges of providing quality services to a diverse population, but they also share our vision in creating endless possibilities centered on meeting the individual needs of those that we serve.

How to become a Service Provider

Quality Assurance (Q.A.) Unit

The Q.A. Unit has several staff whose primary responsibility is to monitor and provide technical assistance to SG/PRC vendored programs, such as residential facilities, adult day programs, and family home agencies.

This unit also serves as the central contact for complaint investigations of vendored programs, including complaints of abuse, unanticipated deaths, staffing issues, program issues, and training issues.   

Transportation Unit

The Transportation Unit serves as a support to our community and SG/PRC staff to provide on-going support with overall processing and coordination of Transportation Service Requests (TSR’s), as well assisting with Bus Pass/TAP Card Enrollment. This unit is available during regular operating hours.

This unit is also responsible for handling complaints regarding transportation services, providing technical assistance to service providers, maintaining routing software and answering general questions about ACCESS services and other transportation services.

Employment Specialist

Our SG/PRC Employment Specialist helps us meet our Employment First Policy.  This specialist is a support to vendors and case management regarding questions about Competitive Integrated Employment, referrals to Supported Employment, and Quality Incentive Programs.

Consistent with state law and its mission our Employment First Policy stresses that employment is a significant way for adults to lead an independent and productive life.

Housing Specialist

San Gabriel/Pomona Regional Center (SG/PRC) has established a strategic plan that includes housing for those we serve as a focus area.  We believe we can best meet our agency’s mission by actively engaging in the development of safe, affordable, and accessible housing throughout the 30 cities in our catchment area.  

We are navigating the affordable housing arena by connecting with Housing Developers who may be able to assist us in developing affordable housing options.  We can help connect developers with state dollars through GAP funding for Multi-Family Housing Projects.

Our Housing Specialist is our contact to these developers and to cities interested in developing housing for the adults that we serve. This specialist is also a support to case management in helping to find resources to assist with evictions, temporary displacement and referrals to housing resources available in Los Angeles County.

Home and Community Based Services (HCBS) Specialists

In March 2014, the Centers for Medicare and Medicaid Services (CMS) adopted updates to federal regulation regarding Home and Community Based Services (HCBS) in order to promote Person Centered Planning, Individualized Services, and Access to the Full Benefit of Community Life.

These services and supports to individuals are with the intent of diverting or transitioning individuals from institutional settings into their homes and the community. Service providers, including those contracted through the Self Determination Program (SDP), are expected to be in compliance upon initiation of services.

Our HCBS/Resource Developers are specialists that assist our agency and the vendor community with understanding the Final Rule.  They also assist with monitoring vendors to meet the Final Rule requirements.  In addition, they assist in developing resources with funding provided by DDS.  These resources include training to individuals and families served, and training to vendors and to agency staff.

Community Resource Development Plan (CRDP)

Each fiscal year, San Gabriel / Pomona Regional Center (SG/PRC) submits its plan to the Development of Developmental Service (DDS) for its Community Resource Development Plan (CRDP). 

The projects we request are based on input from our Community Needs Survey that is sent out to our community each year.

We seek input from individuals served, families, vendors, and SG/PRC staff about which types of services to develop to address unmet needs. We use this input as a guiding tool to determine which CRDP projects to request DDS to provide special funding for its development.

Once our projects are approved, we will post Request for Proposals (RFP) for any projects that are $100,000 or more.

Request For Proposals (RFP)

Our Community Resource Development Plan (CRDP) allows for the development of needed services for individuals served by SG/PRC. SG/PRC will post Request for Proposals (RFP) to identify and recruit qualified service providers to meet the unique needs of individuals served.

During the RFP process each submitted proposal is rated against the guidelines and adherence to the service-related areas identified in the initial RFP posting. This section is designed to list current RFPs and recently awarded proposals.
