Overview

San Gabriel/Pomona Regional Center recognizes that service coordination is the primary means through which SG/PRC carries out our mission. Service Coordinators have the critical responsibility of developing an IFSP/IPP for every individual served and they serve as the point of contact for individuals served, families and service providers.

Early Childhood Development Services (Birth to 5 yrs old)

The Early Intervention units serve children from birth to age three that are at risk for having a developmental disability with coordinating crucial Early Start services.

Our Preschool units serve children from age three to six with the transition from preschool to school services.

Family & Transition Services (6 to 24 yrs old)

The Family Services units serve children ages 3 to 13, who live with their families coordinating supports with school issues, parent training and generic resources.

The Transition Services units serve clients ages 13 to 23, who live with their families preparing them for transition to adult programs.

Adults Services (25 years & older)

The Adult Services units serve individuals 25 years old and above who either live with their families or live independently.

The Residential Services units serve individuals ages 6 and above who live in licensed non-medical or medically focused residential facilities.
