Based on our needs survey and internal information, there is a need to provide specialized residential services to children ages 12 to 17 years of age as there are few Group Homes for Children in our catchment area and also few in LA County. This will meet the need for SG/PRC families and the Department Of Children and Family Services (DCFS). We currently do not have sufficient capacity in our area to address this need. This proposal will allow for the selection of a residential service provider to develop services as a Specialized Residential Facility (SRF – Level 7) with a behavioral or mental health component. In addition, this will be a 4 bed home with at least one room as non-ambulatory

Housing Access Services (Service Code 089)

Regional Centers throughout California are soliciting proposals for the development of Housing Access Services providers that will incorporate the OOMM roommate matching platform into their Individual Housing Transition Services. The OOMM roommate matching service was enhanced to be user friendly for individuals that are deaf or hard of hearing.

Eight Regional Centers will award $50,000 in DDS Community Resource Development Plan start-up funding for eight Housing Access Services providers to support all 21 regional centers.

PROJECT DESCRIPTION

Based on our needs survey and internal information, there is a need to provide specialized residential services to seniors who not only need increased nursing services, but who could also benefit from a program with a behavioral component. Our existing RCFE’s, which are 6 beds or larger, have a nursing component, but none have a behavioral component. This is a unique need in our area. We currently do not have sufficient capacity in our area to address this need. This proposal will allow for the selection of a residential service provider to develop services as a Specialized Residential Facility (SRF), with a nursing component and with an added behavioral service. In addition, this will be a 4 bed home. This home will be licensed as an RCFE and vendored as a SRF.

PROJECT DESCRIPTION

Based on our needs survey and internal information, SG/PRC is in need of an entity to provide training on what it means to be a roommate, including responsibilities, agreements, negotiation, expectations, etc. ( this is for individuals in residential care, those who live on their own or those who are planning to live on their own) . The goal is to offer at least three workshops with goal of vendorization for a future service.

San Gabriel/ Pomona Regional Center (SG/PRC) is seeking proposals from qualified individuals, organizations, or agencies to provide American Sign Language (ASL) training and support services under Service Code 644.

Services provided under this service code are meant to help individuals improve or develop their expressive and receptive communication. The service is intended to help individuals improve functional communication skills through the use of formal ASL and to reduce the use of home signs.

TRAINING SERIES DESCRIPTION
The Self-Determination Program (SDP) is a voluntary regional center program that provides
participants with an individual budget, which they use to purchase the services/activities/items and the
care/support they need to carry out and achieve their person-centered plan (PCP) and Individual
Program Plan (IPP). SDP participants and families may purchase services from traditional providers or
local businesses, hire support workers directly, or negotiate unique arrangements with groups and
individuals in their communities. This type of program allows people with disabilities to be in charge of
their own lives, not the professionals or systems that support them.
In accordance with Welfare and Institutions Code (WIC) Section 4685.8(f), funds have been allocated to
regional centers for the Self-Determination Program (SDP) for Fiscal Year 2024-25, through the A-1
contract amendment allocation on September 6, 2024.

WIC Section 4685.8(f) specifies these funds shall be used to maximize the ability of SDP participants to
direct their own lives and enable the Department and regional centers to successfully implement the
program. The statute further requires the use of the funds to meet the needs of participants, increase
service access and equity, and reduce disparities, and to implement the program, including costs
associated with:
• Employer readiness training/workshops to provide education for participants who are
interested in using the co-employer and sole employer models.
